A hacker gained access to Avenue 360 Health and Wellness through the email accounts of certain employees. Its investigation determined that the accounts were compromised between January 15, 2021, and April 2, 2021, and it was confirmed this was from an email security breach.

According to HIPAA Journal, a total of 12,186 patients in Houston had been compromised. This cost Avenue 360 Health and Wellness an estimated $2,821,044 in operational downtime, legal action, loss of data and reputational damage, according to eRiskHub.

The data breach contained the following protected health information:

  • Names
  • A limited number of Social Security numbers
  • Medical insurance information
  • Dates of birth
  • A limited amount of financial account information
  • Health insurance information
  • Medical information
